Community Mental Health Practitioner

Overview We are seeking a dedicated and compassionate Mental Health Practitioner to join our Community Mental Health Team based at Pathways MHRC in Bamber Bridge. As a Band 6 Keyworker, you will hold a defined caseload and deliver holistic, recovery-focused care to adults with complex and enduring mental health needs. Our service is built on strong values of collaboration, respect, and recovery, and we believe in working in true partnership with patients, carers, and families to form the Triangle of Care -- ensuring everyone involved in a person's care is listened to, supported, and included. Responsibilities Act as keyworker for a defined caseload, delivering assessment, care planning, implementation, and review under the Care Programme Approach (CPA). Work collaboratively within a multidisciplinary team, contributing to shared decision-making and holistic care. Support service users prescribed psychotropic medications, including Clozapine and Lithium, in line with Trust policy and national safety standards. Undertake comprehensive risk assessments and develop collaborative management plans to promote safety and recovery. Deliver psychosocial interventions to promote independence, wellbeing, and recovery. Build and maintain therapeutic relationships with service users, carers, and families, embedding the principles of the Triangle of Care. Liaise effectively with partner agencies, primary care, and voluntary sector organisations to ensure coordinated, person-centred care. Contribute to a positive, learning culture, promoting reflection, innovation, and adherence to professional and Trust values. Qualifications 1 x Professional Qualification – RMN only (Essential Criteria – Pharmaceutical knowledge of medications: particularly psychotropics, Clozapine and Lithium) 1 x Professional Qualification – RMN and/or Social Worker (Desirable Criteria – Pharmaceutical knowledge of medications: particularly psychotropics, Clozapine and Lithium) Additional Skills and Experience A thorough understanding of psychotropic medications, including Clozapine and Lithium, is essential as you will be responsible for supporting safe medication management and monitoring within community settings.
