Role As a Healthcare Assistant (HCA) you will work closely with our multi‑disciplinary team (MDT), which includes a Consultant Psychiatrist, Community Psychiatric Nurses (CPNs), Memory Assessment Nurse, Occupational Therapist, Dementia Liaison Nurse, Psychologist, and the Primary Care Dementia Team. Provide a range of services, including practical and emotional support, as outlined in individual care plans. Work collaboratively with the MDT to ensure high‑quality, person‑centred care. Eligibility This job is currently only open to people who already work for the NHS in Cornwall or the Isles of Scilly. Qualifications A caring, compassionate approach and excellent communication skills. Ability to work as part of a team and independently in the community. Previous experience in healthcare or mental health support is desirable but not essential – full training will be provided. We deliver community and hospital‑based care to improve people's physical and mental health. We also provide specialist support to people with dementia.
