OverviewYou will have opportunities to work closely with multidisciplinary teams in the community, primary care, the acute neuro rehab team, the stroke early supported discharge team and other external agencies. The post is within a multidisciplinary team and you will work alongside Neuro Physiotherapists, Occupational Therapists, Psychologists, Speech and Language Therapists and Rehab Assistants. The post is based at Hinchingbrooke Hospital, Huntingdon with potential to cover across the South of the county and county-wide duties as required. You will work in an agile manner using virtual consultation to manage your caseload, and you will have access to a smart phone and laptop. You will be required to travel independently around the county to meet strict deadlines.We are seeking a dynamic Band 4 Rehabilitation Assistant to support neuro rehab as in-reach to inpatient rehab beds and with patients in their own homes. You will support patients with physical, cognitive and psychosocial challenges as part of a MDT to achieve their potential and improve health, care and outcomes.Staff development is valued, with opportunities for in-house in-service training and a blended competency package to support your professional growth. The ideal candidate will be flexible and adaptable to support service delivery across the organisation.
