There is an in-service training programme in place and an intensive Trust and Local induction will be provided. The post is within a multidisciplinary team and you will work alongside Neuro Physiotherapists, Occupational Therapists, Psychologists, Speech and Language Therapists and Rehab Assistants. The post will be based at Hinchingbrooke Hospital, Huntingdon, however there will be times where they are expected to cover further across the South of the county. The successful candidate may be asked to provide county wide cover at times. The successful candidate will work in an agile manner using virtual consultation to manage their caseload of patients, having access to a smart phone and laptop to support this. Main duties of the job Please note for this job you will be required to travel independently around the county meeting strict time deadlines. We are seeking an enthusiastic and dynamic Band 4 Rehabilitation Assistant to support the delivery of neuro rehab as in-reach to the inpatient rehab beds, as well as working with patients in their own homes. You will play a vital part in supporting patients with physical, cognitive and psychosocial challenges as part of a MDT to achieve their potential whilst instigating and delivering better health, better care and better outcomes for our patients. Staff personal and professional development is viewed as highly important, and staff have the opportunity to receive training through an in-house in-service training programme as well as being supported to undertake a blended competency package to help you develop within your profession and role. The ideal candidate will be flexible and adaptable to support service delivery across the organisation. The post holder must have access to a personal car for independent travel across the work locality. PLEASE REFER TO THE ATTACHED JOB DESCRIPTION AND PERSON SPECIFICATION FOR FULL DETAILS OF RESPONSIBILITIES. • To work autonomously in the community setting under the direction of a registered practitioner within the integrated multi-professional ESD and neuro rehab team. • To work in a ward environment to support patients on the ESD pathway, working closely with the ward Multidisciplinary team on the ward and within the ESD/CNRS. • To be responsible for assessing patient’s physical and functional needs, interpreting, planning and implementing therapy interventions for patients with stroke and neurological problems. • To use clinical judgement to evaluate patient progress, including the recording of objective and subjective measurements. • To manage own caseload of patients who require ongoing management and support with complex needs and to plan and prioritise own workload. • To undertake duties delegated and supervised by a registered professional and following prescribed packages of care. • To keep the patient at the centre of care, ensuring patient and carer participation in decision making. *DVLA have a number of reciprocal arrangements with overseas countries, for further information please visit the DVLA website Person specification Education / Qualifications Educated to minimum GCSE level or equivalent grade A-C, including English RSA/OCR stage II - NVQ Level 3 in health and Social Care or equivalent experience GCSE or equivalent qualification in Maths, Biology and Food Technology ‘A’ level qualification or other post ideally in healthcare Experience Experience of working in rehabilitation in health/social care environment at band 3 level or equivalent Experience of working with people with neurological problems Experience / knowledge of rehab principles Has worked as a Rehabilitation Assistant in the past Experience of working with people with neurological problems Skills & Abilities Able to demonstrate commitment to high quality care and service provision Has a working knowledge of history taking and documentation Ability to record messages and information in an accurate concise way Work flexibly to accommodate patient/service needs Ability to work well within a team as well as individually Good organisation skills (e.g. organising activities, groups, following a plan of care) Observational skills Basic computer skills Proven record of Multidisciplinary working Experience using SystmOne Professional attitude Caring, approachable and courteous manner Team Player Physical Requirements The ability to: kneel, bend & stoop, and work in cramped environments Manoeuvre patients using handling aids Must have access to a personal car and the ability to drive independently around the locality, due to the radius covered and lack of public transport in some area Other Able to work shifts and unsocial hours when required including flexibility to work over seven days per week Recognise peoples right to privacy and dignity, treating every person with respect Willingness to embrace integrated model and new ways of working being flexible in approach and attitude Recognise and upholds Trusts PRIDE values Here at Cambridgeshire and Peterborough NHS Foundation Trust (CPFT) we are dedicated to providing high quality care with compassion.
