We believe in developmental and training opportunities for our staff and are currently seeking highly motivated, passionate Band 5 Community Nurses who aspire to progress into one of the following roles: Band 6 District Nurse – by undertaking the District Nurse Qualification. Chronic Condition Case Manager – by undertaking disease‑specific training to provide evidence‑based care for highly complex needs. Nurse Practitioner – by completing a modular approach to attain advanced clinical skills. Main duties of the job The successful applicant will need excellent communication skills and the ability to provide clinically effective care within the District Nurse Team. The West Division of Betsi Cadwaladr University Health Board delivers District Nursing Services across Anglesey and Gwynedd, working in partnership with organisations to maintain patients in their own homes and communities, avert hospital admissions, empower self‑management, and implement seamless, patient‑centred care. Must be able to travel between patients’ homes both within towns and rural areas in a timely manner. The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply. Qualifications & Person Specification Essential Criteria Registered nurse or within 2 months of gaining PIN. Knowledge of Primary Care. Effective interpersonal and communication skills. Ability to maintain high standards of care. Clinical competency. Evidence of ongoing professional training. Knowledge of Professional codes of conduct. Broad knowledge of relevant community nursing skills and competencies. Working unsociable hours; ability to travel between patient homes in a timely manner. Desirable Criteria Working towards a diploma or degree. Previous community experience. Flexible approach to work. Welsh speaker.
