Overview We are looking for an experienced Mental Health Practitioner to join our Thurrock Primary Care Network (PCN) for a 12-month fixed term. This is reviewed yearly in line with post funding. As part of the NHS long‑term plan, local areas were asked to realign community mental health services with primary care networks, creating new and integrated models of primary and community mental health care. Key Objectives of the PCN Have a key coordinating role at the heart of the GP practices and be a fully integrated member of the multi‑disciplinary team with regards to response and management pathways; Provide a combined consultation, advice, triage and liaison function in accordance with agreed protocols; Support children and young people with mild to moderate mental health problems through evidence‑based interventions. The Mental Health Senior Practitioner will be responsible for the assessment and caseload management of clinicians within their GP PCN. The practitioner will triage cases, complete a full assessment and manage their workstream. The post holder will manage a caseload of CYP with mild to moderate mental‑health needs, support the delivery of evidence‑based low‑intensity interventions in GP services, and liaison with other professionals. Working in partnership with GPs, the post holder will support children, young people and their families in the self‑management of presenting difficulties and will have experience delivering individual interventions and group support. Clinical Responsibilities Work as part of a multi‑disciplinary team in a patient‑facing role to assess and manage patients using expert knowledge; Assist in the assessment of new patients with subsequent production and completion of individual care plans; Provide specialist clinical support and advice to young people with mental health difficulties; Work with patients to support engagement with appropriate services and treatment, which may include onward referral to secondary mental‑health services or sign‑posting to other mental‑health services, voluntary services and support groups; Educate and involve family members and others in treatment as necessary; Be responsible for the day‑to‑day planning of personal workloads; Follow departmental policies, procedures and guidelines; Liaise with the practices and, where practicable, standardise referral processes across the PCN; Triage and receive referrals and directed patients from within the PCN; Communicate effectively with patients, recognising the need for varying methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating. Additional Working Arrangements and Benefits Flexible working arrangements are available and can be discussed on an individual basis after an offer has been made; Retired professionals may return to work on a part‑time basis with shifts that suit them; Training and professional development opportunities with regular supervision and access to other support via NELFT; High‑Cost Area Supplement of 5% of the basic salary (minimum £1,192 and a maximum £2,011 per annum pro‑rated); Induction programme up to two weeks, including mandatory training, systems training and equipment allocation, held at the head office in Rainham, Essex; Ample local travel around the county may be required, with base working primarily from GP practices within the PCN.
