Clinical Pharmacist – Job Summary Have ambition and commitment to provide high quality care to patients, and a passion for clinical development. Working in general practice alongside a team of pharmacists, you will act within professional boundaries and provide support in medicine optimisation and conduct clinical medication reviews. Responsibilities Conduct clinical medication reviews and proactively manage complex polypharmacy patients, reducing inappropriate prescribing where possible. Take responsibility for the management of chronic diseases within the practice. Support the repeat prescription system and manage the repeat prescribing reauthorisation process. Complete medicines reconciliation on transfer of care, liaising with patients and community pharmacists to ensure continuous medication supply. Provide support to practice staff with prescription/medication queries. Support the development and implementation of cost-effective, evidence-based prescribing across the ICB. Provide clinical leadership in medicines optimisation and quality improvement, managing aspects of the quality and outcomes framework and enhanced services. Develop and implement cost management strategies to ensure financial balance is achieved and maintained. Involve the patient in decisions about prescribed medicines and supporting adherence as per NICE guidelines. Recognise priorities when problem‑solving and identify deviations from normal patterns, referring to seniors or GPs when appropriate. Proactively participate in quality improvement and audit activities. Independent prescriber or currently working towards qualification. Minimum of 3 years post‑qualification experience. Desirable Membership of the Primary Care Pharmacy Association (PCPA). Membership of the Royal Pharmaceutical Society. Clinical diploma. Person Specification – Experience Essential In‑depth therapeutic and clinical knowledge and understanding of evidence‑based healthcare principles. Appreciation of the nature of GPs and general practice prescribing. Excellent interpersonal, influencing and negotiating skills. Excellent written and verbal communication skills. Ability to communicate complex and sensitive information clearly to a variety of audiences. Capability to plan, manage, monitor, advise and review general medicine optimisation issues for long‑term conditions. Good IT skills and ability to work effectively both independently and as part of a team. Demonstrated accountability for delivering professional expertise and direct service provision. Desirable Able to obtain and analyse complex technical information. Strong problem‑solving skills, recognising deviations from normal patterns and referring to seniors or GPs when appropriate. Ability to work under pressure and meet deadlines. Produce timely and informative reports. Ability to gain acceptance for recommendations and influence or motivate the audience to comply with agreed actions where there may be significant barriers. Person Specification – Other Essential Meets DBS reference standards and has a clear criminal record, in line with the law on spent convictions. Adaptable and self‑motivated. Completed safeguarding and other mandatory training. Access to own transport and ability to travel across the locality regularly, including to visit people in their own homes.
