Working in general practice alongside a team of pharmacists, they will be required to act within professional boundaries and provide support in medicine optimisation and to conduct clinical medication reviews.Masters qualification is requiredMain duties of the jobTo conduct clinical medication reviews and proactively manage complex polypharmacy patients, and reduce inappropriate prescribing where possible.To take responsibility for the management of chronic diseases within the practiceTo support the repeat prescription system, and to manage the repeat prescribing reauthorisation processTo complete medicines reconciliation on transfer of care which includes hospital discharge summaries, and to liaise with the patient and community pharmacists to ensure a continuous medication supplyProvide support to practice staff with regards to prescription/ medication queries.To support the development and implementation of cost-effective, evidence-based prescribing across the ICBProvide clinical leadership in medicines optimisation and quality improvement and to manage some aspects of the quality and outcomes framework and enhanced servicesDevelop and implement cost management strategies to ensure financial balance is achieved and maintained.Will need to be proactively involved in quality improvement and audit as well as managing some aspects of the QoFRecognises priorities when problem-solving and identifies deviations from normal pattern and is able to refer to seniors or GPs when appropriateInvolve the patient in decisions about prescribed medicines and supporting adherence as per NICE guidelines.About usWe Holyhed Primary Healthcare Centre are a progressive healthcare center dedicated to delivering top-notch patient care and fostering the growth and professionalism of our entire staff. We prioritize the well-being of our staff and the delivery of exceptional care to our patients, embodying a strong sense of compassion.While we work diligently, we also place a strong emphasis on maintaining a healthy work-life balance for our team members.Job responsibilitiesUnderstanding clinical medication reviews, especially with those who are in high risk groups such as: Frail elderly, Polypharmacy, High risk medication & Reviewing pathology results for patients on regular medicationTo reconcile medicines following discharge from hospitals or intermediate care and working with patients and community pharmacists to ensure patients receive the medicines they need post-discharge and reduce risk of readmissionTo be involved in multidisciplinary clinical meetings within PCN where appropriateTo manage repeat prescription requests, resolving queries where possible within scope of practice. To ensure patients have appropriate monitoring tests in place when requiredImplement changes to medicines in line with MHRA alerts, product withdrawal or shortage and other local or national guidanceContribute to the provision of out of hours/on call/ extended services for the practice and the patients. These can include patient facing and telephone consultation,Signposting to other services/ or healthcare professionals where appropriate, while working within scope of practice and limits of competencyParticipate in review and setting of policies relating to medicines management and prescribing and help ensure practices prescribe in accordance with local guidelines and formularyUndertake a proactive role in audit and quality improvement implementing recommendation where appropriateTo monitor network practices prescribing against the local health economy and make recommendations for GPs or amend within scope of practice, based on findingsTo provide independent information to patients with regard to medicines and prescribing changes, initiating further support from other healthcare professionals where appropriateProvide assistance in completing structured medication reviews and completing targets in relation to the investment and impact fund (IIF) schemeTo maintain full and complete records of all patients contacts using the appropriate clinical templates and codingTo maintain own clinical and professional competence and be responsible for own continuous professional developmentTo identify areas of Clinical Risk at Network level and make recommendations to support the introduction of new working practices that will optimise the quality of prescribing and make more efficient use of network resourcesWork with practices to ensure full compliance with Care Quality Commission standards for safe and effective careTo take an active role in development and embedding of the PCNs culture, values and reputation as providers of high quality servicesTo work as a flexible member of the PCN providing support to other team members when necessaryPerson SpecificationQualificationsMandatory registration with the General Pharmaceutical CouncilIndependent prescriber or currently working towards qualificationMinimum of 3 years post-qualification experience.Membership Primary Care Pharmacy Association (PCPA)Membership of the Royal Pharmaceutical SocietyExperienceIn depth therapeutic and clinical knowledge and understanding of the principles of evidence- based healthcare.An appreciation of the nature of GPs and general practicesAn appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribingExcellent interpersonal, influencing and negotiating skillsExcellent written and verbal communication skillsDemonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences (e.g. patients)Is able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions.Work effectively, independently and as a team memberDemonstrates accountability for delivering professional expertise and direct service provisionAble to obtain and analyse complex technical informationRecognises priorities when problem solving and identifies deviations from the normal pattern and is able to refer to seniors or GPs when appropriateAble to work under pressure and to meet deadlinesProduce timely and informative reportsGain acceptance for recommendations and influence/motivate/persuade the audience to comply with the recommendations/agreed course of action where there may be significant barriersOtherMeets DBS reference standards and has a clear criminal record, in line with the law on spent convictionsAdaptableSelf MotivationSafeguarding and other mandatory trainingAccess to own transport and ability to travel across the locality on a regular basis, including to visit people in their own homesImmunisation statusDisclosure and Barring Service CheckThis post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions. #J-18808-Ljbffr
