OverviewJob Title: Community Learning Disability Nurse / RMNBand 6: Full Time: 37.5Base: Community Learning Disability Service Mile End Hospital London E1 4DGThe Community Learning Disability Service provides integrated specialist health and social care to adults with learning disabilities in Tower Hamlets. Additionally the service is responsible for the provision of a specialist day activity project - CREATE.ResponsibilitiesTo use nursing skills in working with people with learning disabilities and autism to maintain, promote and improve health.Be able to assess and support people with learning disability and complex health needs, autism and commonly coexisting conditions like ADHD and mental illnesses.Be able to fully involve the person with a learning disability who is at risk of behaviors that challenge.To support people with learning disabilities to develop their full potential by reducing the impact of their impairment and promoting social inclusion.To facilitate access to health services through collaboration and partnership with primary and secondary health, and other relevant services.To use a person-centred approach to promote choice, independence and inclusion.To contribute to shaping local health services in the way they are organised and run to promote an inclusive service for people with learning disabilities.The role involves undertaking assessments, reviews and care planning for NHS Continuing Healthcare Funding in accordance with the National Framework for NHS Continuing Healthcare.Understand the implication of the legal and policy framework underpinning the support of people with a learning Disability and autism.Key working relationshipsThe post holder is line managed by the Care Team Manager (Integrated Health & Social Care) – Integrated Multi-professional Team.Post holder is professionally accountable to the Clinical Nurse Specialist (Lead Nurse).Maintain multi professional working relationships within CLDS, Primary and secondary care services and other agencies that support people with learning disabilities in Tower Hamlets.Additional informationThe post falls within the Criminal Records Disclosure Policy and therefore a criminal record check will be applied for successful candidate.For further information regarding the post kindly contact: shabnam boodhoo 02077715500Kindly refer to the job description and person specification that is attached to the vacancy profile.This advert closes on Thursday 18 Dec 2025 #J-18808-Ljbffr
