Advanced Nurse Practitioner – NHS Scotland The Acute Frailty team at Victoria Hospital, Kirkcaldy, is seeking an Advanced Nurse Practitioner to improve the care and experience of people living with frailty in the acute hospital setting. Key Responsibilities Provide early identification, effective assessment, and management of frailty in front‑door areas (Admissions, ED, GP Assessment). Prioritise care and provide daily medical reviews of patients with frailty alongside a multidisciplinary team. Facilitate timely identification of patient pathways and ensure appropriate destination of care. Support the development of the Acute Frailty Unit and coordinate care between front‑door areas and the unit. Communicate with patients and carers empathetically and clearly, particularly around future care planning. Qualifications & Experience Qualified Nurse or registered AHP with significant acute care and frailty experience. Non‑medical prescribing and clinical decision‑making qualifications (or a clear plan to obtain them). Evidence of post‑registration professional practice at masters‑degree level or suitable postgraduate training. Desire to develop within a dynamic service; interest in becoming an Advanced Practitioner.
