To be responsible for the implementation of processes for theeffective management of patients with long-term conditions using evidence-basedpractice including care for elderly and housebound patients.Working as a senior member of the practice multidisciplinary team,the post holder will ensure nursing services are delivered effectively to theentitled patient population.The Practice Nurse will be responsible for a number of clinicalareas such as infectionprevention and control, health promotion, chronicdisease management, health promotion, well women and well man clinics, as wellas actively supporting the practice management team in the reviewing anddelivery of clinical policy and procedure.Village Surgery must ensure that the post holder hasaccess to appropriate clinical supervision and an appropriate named individualin the organisation to provide general advice and support on a day-to-day basisMain duties of the jobThe Practice Nurse plays a key role in delivering safe, effective, and evidence-based care while supporting practice policies and patient wellbeing. Core duties include supporting the Infection Prevention Control lead, audits, training, and compliance, and contributing to the annual IPC statement. The role involves developing and embedding care management programmes for frail and housebound patients, implementing health promotion and wellbeing initiatives, and managing chronic disease clinics. Responsibilities include assessing patient needs, creating and evaluating treatment plans, diagnosing and managing acute and chronic conditions, prescribing and reviewing medications, and advising patients on safe use. The nurse co-ordinates travel medicine, requests and processes pathology, and ensures accurate clinical records, coding, and maintenance of chronic disease registers. They deliver and support well woman/man clinics, vaccination programmes, safeguarding, and opportunistic health promotion. Additional duties include assisting in minor surgery, prioritising health issues, managing clinical emergencies, recognising and referring mental health needs, and supporting vulnerable patients through liaison with external services. Delegation, supervision, and safe practice within the team are essential elements.In addition to primary responsibilities, the Practice Nurse may contribute to wider practice development. This includes developing and reviewing audit protocols and nursing protocols, supporting nursing colleagues through guidance, training, appraisals, and rota coordination, and leading service improvements. The nurse participates in local initiatives, shared learning, and policy implementation to enhance care. They review clinical practices in line with national policy, contribute to significant event analysis using structured approaches such as root cause analysis, and support the continuous improvement of protocols and service delivery.
