The Triage Nurse role is integral to ensuring that all patient requests into the practice are clinically assessed and prioritised accordingly. The post holder will provide self-care advice appropriate to the patients needs or allocate them to the most appropriate practitioner or service.Main duties of the jobWorking as part of the Triage team, the post holder will provide clinical triage of all e-consultations, telephone calls and home visit requests where needed.The post holder will identify the clinical need and urgency to provide a time frame for intervention, allocating them to the most appropriate clinician or service to meet the needs and will provide advice and signposting as required.There may be, on occasion, a requirement to carry out other tasks.About usThe Park Medical is a progressive, patient-centred GP surgery located in Derby, working over 3 sites, serving approximately 28,000 patients. Our practice is proud of its welcoming culture, strong values, and commitment to continuous learning.Our current team includes: 8 GP partners, 8 Salaried GPs, 5 ACPs, 4 Pharmacists, 2 Pharmacy Technicians, 2 Social Prescribers and 1 First Contact Physio.We have a workflow team to minimise document processing.Job responsibilitiesThe post holder will utilise clinical triage and assessment skills, whilst working within the Scope of Professional Practice and adhering to local policies/guidelines.
