Join to apply for the Senior Community Occupational Therapist role at Sirona care & health. Overview We are recruiting an enthusiastic, highly motivated Senior Occupational Therapist to work within our Integrated Neighbourhood Teams (INT) in South Bristol. You will deliver community‑based therapy, prevent unnecessary hospital admissions and support rehabilitation in patients’ home environment. Responsibilities Deliver community‑based therapy to improve quality of life and prevent hospital admissions, providing effective rehabilitation in the home environment. Conduct initial holistic assessment and implement and evaluate programmes of care for patients with acute and chronic conditions. Use clinical reasoning to assess, plan, implement and evaluate evidence‑based, patient‑centred interventions. Manage your own caseload, coordinate the service and ensure efficient team coordination. Develop and maintain strong, communicative working relationships to prevent avoidable admissions and facilitate timely discharges. Collaborate with service users, carers and family to identify achievable goals and maximise independence and quality of life. Facilitate appropriate referrals to other services, arrange equipment within scope of practice, and report any issues. Take responsibility for the development of other INT members, including peers, junior staff and students. Maintain accurate, informative and legal records. Actively participate in training activities and assist in delivery. Identify risks and act to minimise risk to yourself, colleagues and service users. Benefits Agenda for Change salary.
