We work in partnership with parents, education services, and other professionals in education and health.The Early Years and Disability Inclusion Team monitors and supports schools/settings to successfully include all children with emerging and/or additional needs and disabilities to access their entitlement, environment and make progress.About the jobAn exciting opportunity has arisen for a Transition Worker, working as part of the Disability and Early Years Inclusion Team within the Education Service and with partners in many different agencies.
