Transitional Care Support Worker Band 3
The Transitional Care Unit is looking for experienced Health Care Support Workers passionate about promoting patient's independence to join the Multi Disciplinary Team in St David's Hospital.
Supporting the MDT to facilitate timely hospital discharge and prevent deconditioning often associated with extended hospitals stays.
The primary focus of this new unit is to effectively facilitate timely discharge, in a cohort of patients who have finished their acute medical/rehabilitation journey. These patients are waiting for their ongoing care needs to be met in a range of care settings in the community.
