Community Staff Nurse – North Tees and Hartlepool NHS Foundation Trust Posted 26 November 2025 Job Summary We are inviting applications for a highly motivated, dynamic staff nurse to join our Community Nursing Service. The role will provide care to patients in a range of settings, including patients’ own homes and residential care homes. The post holder should have a solid knowledge of community care and a particular interest in palliative care, long‑term conditions, and wound care. You will work autonomously to deliver a whole‑person approach to the care and management of patients within these settings. Responsibilities Provide care to patients in their own homes and residential care homes. Deliver high‑quality, individualized and holistic care. Work closely with local authority services and specialist nursing teams. Specialty areas include palliative and end‑of‑life care, wound care, diabetes management and continence management.
