Adolescent Outreach Senior Case Worker and Social Worker

Job Overview We are looking to recruit an experienced band 7 Social Worker to join the established Adolescent Outreach Team (AOT) within the South West London and St George’s Mental Health NHS Trust. The team comprises a highly skilled multidisciplinary group committed to supporting young people, their families and carers to remain at home, and to facilitate safe discharge and reduced length of stay when hospital care is required. Key Responsibilities Work within AOT as a Senior Case Worker, providing care coordination and assessment of an agreed caseload of AOT clients, and serve as the primary Social Worker for the Social Work function in AOT. Be committed to effective team working and coherent delivery of care, including individual, family, group work and telephone support. Develop the service to meet the needs of young people and their parents/carers, and lead in this area. Liaise and interface with multidisciplinary staff within the service area. Liaise and work with external agencies, especially Local Authority Children’s Services and Tier 3 CAMHS. Investigate and advise on complex safeguarding cases and other high risk social factors. Produce written reports for a variety of audiences, including social circumstances reports for Mental Health Tribunals, safeguarding investigation reports, social work reports, periodic audits, and analytical reports for the Directorate and Head of Social Work. Provide consultation to other services and agencies, including clinicians and multi‑disciplinary teams. Supervise less experienced social workers in day‑to‑day clinical practice. Maintain up‑to‑date knowledge of relevant legislation, guidance, Trust policies and procedures. Maintain high standards of professional practice, working in the best interests of service users and carers. Work closely with all relevant service members to achieve best possible outcomes and promote the interests and welfare of patients and families. Lead on adult and child safeguarding within the service area and act as a key liaison between the team, Service and local/regional statutory safeguarding and child protection services.
