South West London and St Georges Mental Health NHS Trust Adolescent Outreach Senior Case Worker and Social Worker The closing date is 10 December 2025. We are looking to recruit an experienced band 7 Social Worker to join our established Adolescent Outreach Team. The multidisciplinary team is skilled and highly committed to supporting young people to remain in the community and to facilitate safe discharge from hospital when necessary. Our team retains staff at high levels and works closely with local inpatient services (Aquarius, SLP units), Tier 3 community CAMHS, CAMHS Emergency Care Team, Crisis Line and Bed Management. Location: Springfield University Hospital, Tooting. Main Duties The post-holder will work within AOT as a Senior Case Worker providing Care Coordination and Assessment of an agreed caseload of AOT clients and will have the primary Social Worker role servicing the Social Work function in AOT. They must be committed to effective team working and coherent delivery of care, including individual, family, group work and telephone support. The role will be at the forefront of developing the service to meet the needs of young people and their parents/carers. They will maintain strong working relationships with local inpatient services, SLP inpatient units (Snowsfields and BAU), Tier 3 community CAMHS, CAMHS Emergency Care Team (CECS), CAMHS Crisis Line, and CAMHS Bed Management. Job Responsibilities To liaise and interface with multidisciplinary staff within service area; work with external agencies, including Local Authority Children’s Services and CAMHS Community Services (Tier 3); investigate and advise on complex, sensitive and confidential safeguarding cases; provide written reports for various audiences (Mental Health Tribunals, Managers Hearings, safeguarding investigations, social work reports, audits, analytic reports); develop and provide consultation to other services and agencies; supervise less experienced social workers; maintain up‑to‑date knowledge of legislation and Trust policies; maintain high standards of professional practice in the best interests of service users and carers; work closely with all members of relevant service to achieve the best possible outcomes; promote the interests and welfare of patients and families; lead on adult and child safeguarding within service area and act as key liaison between the team, local and regional statutory safeguarding and child protection services, and partner agencies. Person Specification Experience Significant experience of statutory LA social work Experience of specialist assessment and treatment of young people with serious mental health problems who are in crisis Experience of working with adolescents and families with highly complex mental health needs, high levels of emotional distress, and risk‑taking behaviours Experience of undertaking risk assessment and risk management, and the ability to analyse and make judgements and decisions about treatment plans Experience of working in a community environment Experience in delivering psychological therapies Training & Qualifications Recognised social work qualification or equivalent Current registration with Social Work England (SWE) register of qualified social workers Evidence of relevant post‑qualifying formal study / professional development Completion of post‑qualifying award or advanced award in social work Family Therapy/Family Work training Disclosure and Barring Service Check This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and it will be necessary for a submission for Disclosure to the Disclosure and Barring Service (formerly CRB) to check for any previous criminal convictions.
