We have a fantastic opportunity for an experienced Intensive Outreach Case Worker to join our friendly Community Engagement Team in Westminster. The role will focus on enabling people with learning disabilities to engage meaningfully with their communities and build practical life skills through this short-term intensive support engagement. About the role: The Intensive Outreach Case Worker will manage a case load of people with learning disabilities and/or autistic adults who require intensive support to achieve their identified needs. The post holder will work to support people with their social, advisory, employment and cultural needs. We require an experienced, dedicated and skilled Intensive Outreach Case Worker who can use strengths-based goal setting to achieve successful outcomes with the people we support. The role will provide you with an excellent opportunity to develop your potential, learn new skills and make a real and positive change to the lives of the people who use our services. The role will provide practical goal-oriented support in the community, such as assisting with travel training, accessing local services, exploring opportunities for learning, employment and social engagement. Applicants will need to have experience of working with people with Learning Disabilities, Mental Health, Autism or PMLD (profound & multiple disabilities). The successful applicant will be working in partnership with our services users, families, professionals and other organisations; therefore, you must be highly organised with strong communication skills. About you: The ideal candidate will be a well organised person with the ability to deliver successful outcomes for the people we support. Applicants must be able to demonstrate the ability to develop strong relationships with a wide range of stakeholders. As a self-starter, you will be able to use your own initiative, can problem solve and prioritise especially when dealing with emergency situations, good planning and organisational skills. In addition, you will need a positive, resilient attitude, be able to work under pressure, meet needs and be flexible and adaptable. Candidates should: be computer literate be able to work flexibly which may include some weekend or evening work be able to work ‘hands on’ within a team setting have experience in supporting adults with learning disabilities to build skills and confidence in community settings including face to face outreach and local networking. have evidence of continuing professional development within social care roles Experience of being a case worker and using strengths-based goal setting with clients (Desirable) This is an excellent opportunity for someone who is looking for a new challenge We offer continuing opportunities for learning and development together with regular supervisions and support for all our staff.
