Early Intervention in Psychosis Care Co-ordinator – Band 6 Band 6 Early Intervention in Psychosis Team – North Warwickshire & Rugby Full time – 37.5 hours per week Care Coordinator We have an opportunity for a Nurse, Occupational Therapist or Social Worker to join our well established Early Intervention team as a Care Coordinator. We work with individuals who are experiencing the first onset of Psychosis. Responsibilities Conduct a detailed initial assessment incorporating the CAARMs assessment to identify at‑risk mental states (ARMS) or first episode psychosis (FEP) and formulate a care plan for the next 3 years. Collaboratively identify the individual’s goals and needs and support them towards achieving these within a Recovery philosophy. Support clients with housing, finances, vocational activities and social inclusion, working creatively with team members and independently to achieve goals. Support people to independence so they no longer need input from Mental Health Services. Qualifications Registered Nurse, Occupational Therapist or Social Worker with experience in Early Intervention or Psychosis Care. Strong assessment and formulation skills with knowledge of ARMS/CAARMs and FEP. Excellent communication and collaborative skills to work within an interdisciplinary team.
