A Vacancy at Coventry and Warwickshire Partnership NHS Trust. Care Coordinator Band 6 Early Intervention in Psychosis Team North Warwickshire & Rugby Full time 37.5 hours. We have an opportunity for a Nurse, Occupational Therapist or Social Worker to join our well established Early Intervention team as a Care Coordinator. We work with individuals who are experiencing the first onset of Psychosis. Early Intervention in Psychosis is an innovative and evidence-based model. The teams work with service-users aged 14 to 65 who are experiencing first episode or early psychosis, and their families, for up to 3 years, as well as people presenting with an at-risk mental state. There is a strong commitment to provide an excellent standard of care and to continually improve what we do. The NHS England Access and Waiting Time Standard has led to significant development within the team. We are undertaking specific skills training with the clinicians to support both the clients’ recovery and the achievement of these targets. The role involves a detailed initial assessment incorporating the CAARMs assessment to identify at risk mental states (ARMS) or first episode psychosis (FEP). This is incorporated into a formulation to plan the client's care over the next 3 years. The team uses a Recovery philosophy to collaboratively identify the individual’s goals and needs and to support them towards achieving these. Many of our clients experience difficulties with housing, finances, vocational activities and social inclusion and the successful applicant will be expected to work creatively, both with other team members and independently to support the client achieve their goals.
