Neighbourhood Staff Nurse

The post-holder will be a member of the Neighbourhood Nursing (District Nursing) Team and will participate in the clinical case management of patients with a wide variety of complex, high-risk and supported/self‑care conditions. Working directly with the Team Leader, the post-holder will help ensure the provision of a high standard of nursing care and health promotion. Care will be provided in a range of settings including patients' homes, residential homes, nurse‑led clinics and GP practices. Responsibilities Holistic assessment, planning, implementation and evaluation of a plan of care for patients with long‑term conditions, tissue viability, continence, palliative care, and promoting and facilitating patient independence. Assist in the delivery of National Service Frameworks. Provide public health and health promotion programmes to improve health and reduce inequalities.
