These newly developed posts, based within the beautiful area of Conwy & Denbighshire in our newly developed Central Hub, will provide caring clinicians with flexibility, autonomy and professional growth and development. Job Overview As part of our transformation and development of Child and Adolescent Mental Health services, we are strengthening our leadership, pathways and services to provide a rapid, robust and extended multi‑agency crisis response that includes prompt assessment, liaison, consultation, training and where appropriate intensive time‑limited interventions for children and young people up to the age of 18 who are experiencing an acute mental health and/or psycho‑social crisis. Main Duties Undertake assessments in A+E, acute wards and other community settings to support rapid crisis response in least restrictive ways, ensuring safety for young people and others. Provide clinical formulation, identifying the young person’s and family’s needs, and offer liaison, consultation and intervention to facilitate safe discharge and care planning across partner agencies. Work flexibly across North Wales, including out of hours, to support a 24‑hour provision. Be part of a multi‑disciplinary regional crisis service, supporting young people experiencing mental health crisis, reducing need for hospital admission when possible. Provide urgent assessment and brief evidence‑based intervention; support parents, carers and front‑line professionals with education and advice on managing the condition. Maintain good links with other CAMHS services and relevant agencies. Travel within the region as required. Essential Criteria Registered as a Nurse (NMC), Social Worker (Care Council), Occupational Therapist (Health Professions Council) with evidence of personal and clinical development. Post‑graduate training for supervision and teaching of junior staff. Extensive knowledge of mental health issues. Up‑to‑date knowledge and application of consent, data protection, patient confidentiality, risk management and health & safety. Working knowledge of the Mental Health Act and relevant national strategies. Knowledge of the principles of change management. Evidence of organisational, communication and basic IT skills. People‑management and leadership skills; analytical and effective problem‑solving abilities. Advanced clinical skills and ability to use de‑escalation and break‑away techniques (subject to training). Willingness and ability to travel and work a 7‑day/night pattern. Desirable Criteria Degree in a relevant clinical/professional area, specialist practitioner award. Knowledge of interview and appraisal processes, management theories, local policies and guidelines. Experience of resource management and policy development. Experience of working as a mental health practitioner in a community setting. Welsh speaking. Ability to effect positive change. Experience of learning and leading change initiatives. Experience Relevant post‑registration experience working with children, young people and families in health or social care or mental health settings. Experience in completing mental health assessments and risk management. Values Team player; positive role model influencing and sharing skills and knowledge. Enthusiastic, highly motivated and capable of motivating and influencing others. Use own initiative in complex situations. Commitment to community service provision.
