You will provide expert, patient-centred palliative care, support individuals with life‑limiting illness, and lead advanced assessment, planning, implementation, and evaluation of care plans across acute and community settings. As a clinical expert you will support patients, families, and colleagues to deliver compassionate, evidence-based, person‑centred care. You will lead on the development and implementation of our education strategy across the organisation, deputise for the SPCT Leader as required and promote a culture of continuous learning and improvement. Main dutiesProvide expert clinical leadership and advanced palliative care to patients with complex needs across acute and community settings. Act as a role model and source of specialist advice to nursing and multidisciplinary teams.Complete advanced assessments, develop and evaluate individualised care plans, and use non‑medical prescribing where appropriate to ensure timely and effective interventions.Lead the development, delivery, and evaluation of a Trust‑wide education strategy for palliative and end of life care. Design and facilitate learning programmes for all staff groups and contribute to post‑graduate teaching and training.Work with SPCT to develop nurse‑led initiatives, implement evidence‑based practice, and lead on quality improvement projects that enhance patient outcomes and experience.Provide day‑to‑day leadership within the SPCT and deputise for the Team Leader when required.
