Paying £37 per hour. *You will be based at home, but must be able to complete visits in person*Working withina community multidisciplinary Health and Social Care Learning Disability Team. Experience of working with people who have an LD is desirable.Social Worker will be required to complete Care Act assessments and appropriate care and support plans, MCAs, CommunityDoLS applications and Duty.
