Accessing Services Provide advice, information, and signposting Assess urgency of referrals and initiate enquiries Safeguard vulnerable individuals and ensure accurate record-keeping Transfer information to partner agencies in a timely manner Assessment of Need Conduct holistic, person-centred assessments Collaborate with individuals and carers to agree personal outcomes Explore creative solutions to maximise independence and choice Arrange equipment, adaptations, and inter-agency support Record unmet needs and complete documentation in line with statutory guidance Attend case conferences and inter-agency meetings Prepare specialist reports, including court documentation when required Requirements: HCPC registration Occupational Therapy Degree Experience in adult social care and community-based work Willingness to participate in a 365-day rota Ability to work independently and flexibly Access to a vehicle is desirable This is a hybrid role, with most work done remotely or in the community. Team members are encouraged to attend the office once per week to maintain collaboration and team cohesion.
