Job overview We are looking for a motivated and enthusiastic pharmacist to work alongside our Consultant Pharmacist in Nutrition as part of the nutrition support team which works across our Churchill and John Radcliffe hospital sites.OUH is a nationally designated Intestinal Failure centre for both type and type patients.
This role entails leading on the pharmaceutical aspects of adult clinical nutrition for both inpatients and patients requiring nutrition at home.Applicants will have completed a post-graduate diploma and be able to demonstrate solid foundation level practice with experience of adult parenteral nutrition.
Applicants that do not meet all essential criteria may be considered for a Band to progression within this post.
Main duties of the job Clinical Practice Role: To maintain a clinical expertise and develop particular areas of interest as appropriate and in accordance with the Trust and Pharmacy strategies.
You will be providing and receiving highly complex information often in situations where there may be barriers to understanding.
Requirement to communicate highly complex drug or medicine related information to prescribers, clinicians, patients: patients may have language difficulties, physical or mental disabilities and other professionals may challenge advice.
To provide a ward-based clinical pharmacy service according to Trust agreed standards including: This role involves having access to dedicated office workstations but the majority of ward rounds will be conducted in clinical areas such as wards and intensive care units.
Actively participate as a member of daily inpatient PN ward rounds that take place on different hospital sites, including patient assessment involving for example independent clinical assessment of patients and establishing highly complex facts requiring analysis, interpretation and comparison of options.Will require skills and knowledge for analyzing drug, patient specific information in clinical nutrition to provide advice on medicines, dosages, formulations, and production issues.
May also have to make decisions in areas where information is lacking and medical or other opinion differs.
Attend and support nutrition MDT meetings, for both inpatients and outpatients.
