Job summary The role will bridge between HCSWs and the Registered Nurse (RN), within the discipline of nursing.The role will provide support the RN by caring for patients, enhancing the quality of hands on care.The post holder will provide high level support in the provision of quality interventions to meet the needs of patients requiring nursing and clinical care, as an RN assistant.To provide clinical support to implement planned care to patients under the indirect supervision of Registered Nurses.To work within a designated team but with the ability to work independently as required.To demonstrate an awareness of the limits of personal professional practice and knowledge and recognise when to seek appropriate support/adviceTo carry out nursing care, clinical observations, wound care and point of care testing.To undertake clinical tasks cannulation, venepuncture, ECG, wound care.To supervise Band 2 & Band 3 Healthcare Support Workers, delegating care as appropriate.To participate in case conferences and case review meetings.To keep clear, accurate, contemporaneous patient records of the care provided and its impact.
Main duties of the job The Assistant Practitioner will provide high level support to the RN in the provision of quality care and interventions to meet the needs of patients, requiring clinical care within the hospital/ward environment and will meet following requirements:To effectively communicate with patients and families, utilising tact, empathy, compassion and reassurance where there are barriers to understanding, learning disabilities or language difficulties or where English is not the first language or there are sensory deficits.To communicate with the wider MDT, sharing information as required.Update the RN regarding interventions and information as required, test results.Receive and communicate sensitive patient related information, abiding by GDPR regulations.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
