For an informal discussion please contact Marc Willoughby, Pharmacy Team Lead, or Tessa Goodchild, PCN Manager, Main duties of the job The post holder will work as part of a multi-disciplinary team in a patient-facing role across the North Kerrier Primary Care Network, working under the supervision of the Team Lead and GP Lead at each practice.The purpose of the role is to lead improvements to maximise safe, cost effective best practice in prescribing to improve the quality of patient care.
You will help patients to get the best from their medicines by switching medications to agreed and approved protocols, improving repeat prescribing processes in General Practice, including promotion of repeat dispensing and online ordering, minimising clinical risk and aiming to reduce wasted medicines.
You will be able to manage your own workload to provide a broad range of services to fully utilise the expertise a Pharmacy Technician can bring to our Primary Care Team.The post holderwill be the first point of contact for non-clinical medication related queriesfrom pharmacies and other health care professionals, to improve the accuracy ofpatient medication records.The post holderwill be responsible for reconciling all medication change requests fromhospital letters and discharge summaries or as directed by GPs, plus audits.

Job description Job responsibilities PLEASE SEE ATTACHED JOB DESCRIPTION AND PERSON SPECIFICATION AND COMPLETE THE APPLICATION FORM ATTACHED AND SEND TO Undertake patient facing and patient supporting roles to ensure effective medicines use, through shared decision-making conversations with patients.
Support as determined by colleagues, medication reviews and medicines reconciliation for new care home patients and synchronising medicines for patient transfers between care settings and linking with local community pharmacists.
Undertake medicines optimisation tasks including effective medicine administration (eg inhaler technique), supporting medication reviews and medicines reconciliation.
Where required, utilise consultation skills to work in partnership with patients to ensure they use their medicines effectively.
Provide specialist expertise, where competent, to address both the public health and social care needs of patients including lifestyle advice, service information and help in tackling local health inequalities.
To be involved in the production and review of prescribing guidelines and policies where indicated.
Support the implementation of national prescribing policies and guidance across the PCN, care homes and other primary care settings.
Undertake clinical audits supporting quality improvement measures and contributing to the Quality and Outcomes Framework and enhanced services.
Assist in the delivery of medicines optimisation and management incentive schemes and patient safety audits.
Take a central role in clinical aspects of shared care protocols and liaising with specialist pharmacists for more complex patients.
Work with the PCN multi-disciplinary team to ensure efficient medicines optimisation, including implementing efficient ordering and return processes to reduce wastage.
Support the PCN with streamlining the general prescription requests.
Provide training and support on the legal, safe and secure handling of medicines, including the implementation of the Electronic Prescription Service (EPS).
Promotion of Electronic Repeat Dispensing (eRD) and online ordering.
Person Specification Experience Essential Experience of working in a pharmacy setting.
Be able to practice and prescribe safely and effectively in a primary care setting.
Qualifications Essential Registered with the GPhC Enrolled in, undertaking or qualified from an approved training pathway.
BTEC/NVQ level 3 or equivalent in pharmaceutical sciences; Computer literate with an ability to use Microsoft office packages; Accurately, identifying errors quickly and easily able to effectively manage allocated resources; Has a planned and organised approach with an ability to prioritise their own workload to meet strict deadlines; Able to think analytically; anticipating obstacles and thinking ahead; using analytical techniques to draw logical solutions to problems; Excellent communication skills, verbal and written, with the ability to adjust communication style and content to suit the audience; Influencing and negotiating skills; An excellent understanding of data protection and confidentiality issues.
Desirable Ability to use the required GP Clinical System.
