Main duties of the job The Senior Community Mental Health Practitioner will work as a core member of the Community Mental Health Team to co-ordinate the care of a complex caseload of service users, supervising Band 5 Community Practitioners and Band 3 Community HCSW's where appropriate.Working under the supervision of the Deputy Team Manager/Team Manager the practitioner will formulate care and risk management plans in accordance with identified need and ensure that these are recovery focussed and developed in collaboration with the service user and where appropriate their relative and carer.
