Job Title: Health Advocate

Work as a health advocate in supporting patients with Multiple Sclerosis (MS) in the community across Fareham, Gosport, Havant, Waterlooville, Petersfield, and Hayling Island.

Main Duties of the Job

To deliver a specialist service providing specialist case management and educational support for patients with Multiple Sclerosis (MS) under the supervision of the Multiple Sclerosis Specialist Nurse Lead.

Key Responsibilities:

  • Coordinating care with partner agencies and key stakeholders to promote well-being and education for patients and healthcare professionals.

Requirements:

  • Maintaining registration on the Health Professions Council or NMC.

Salary: £43,308 per annum based on full-time hours.
