About us Robin Lane Health & Wellbeing Centre is a GP surgery with a Wellbeing Centre and AQP Eye Clinic based in Pudsey, Leeds.
We are a three GP partner training practice serving a population of 13,800 patients, and we use SystmOne.
We offer training for medical students, nursing students and GP registrars.
We are a forward thinking, multi-disciplinary team and we work supportively with our patient facing Care Navigation team.
We have a modern purpose-built practice building and we are rated Outstanding by the CQC.Robin Lane Health & Wellbeing Centre are an active member of the West Leeds Primary Care Network.
Job description Job responsibilities Core Responsibilities1.
Working within the practice based team to undertake medication reviews particularly in high risk groups such as:- Frail elderly- Poly-pharmacy- Renal impairment- Hepatic impairment- Substance misuse- Patients on high risk medicines- STOPP/START identified patients- Revolving door Hospital admissions2.
Improve prescribing practice through educational support for all prescribers within the practice3.
Lead on where changes in evidence require changes in prescribing across patient population where a drug is withdrawn or indications change4.
Liaise with colleagues in community pharmacy to align support for medicines adherence such as MUR and NMS5.
Support improvements in clinical care through practice based audit and implementing change6.
Prescribing advice to prescribers in practice temporary non availability of drugs7.
Ensure patient safety when they are transferred between care providers through reconciliation of prescribed medicines as a source of medicinesinformation for all of the practice team and patients ( around doses, sideeffects, adverse events, possible alternatives around out of stocks) daily Pathologyresults for patients on known medicines10.Implement drug withdrawalsand alerts MHRA aimed at improving medicines safety11.

Collaborative working with local pharmacies, PCN, ICB and secondary care familiarwith and adhere to the practice safeguarding policies, procedures, andguidelines for both children and adults at risk of abuse or neglect.
This mustbe in conjunction with the Multi-agency Safeguarding Children and SafeguardingAdults at risk of abuse or neglect Policy, Protocols, and guidelines.employees should undertake mandatory safeguarding children, adults and MCAtraining (as appropriate to role) including updates and other professionaldevelopment activities to ensure training compliancy.
Person Specification Qualifications Essential Completion of an undergraduate degree in Pharmacy, and registration with the General Pharmaceutical Council Minimum of 2 years post graduate experience in pharmacy, as demonstrated within a practice portfolio May hold or be working towards a prescribing qualification May hold or be working towards a postgraduate pharmacy qualification Experience Essential COLLABORATIVE WORKING RELATIONSHIPS 1.
