Job summary We are looking to recruit a clinical Supported discharge Matron to be part of the BHFT East Supported discharge service.
The role will be based in WPH working with the acute discharge team you will be expected to carry out face to face assessment within the acute as well as work with the hospital MDTs, including physicians, dietitians, and specialist nurses, to offer a joined up, integrated service.
This is a Band 7 Permanent Post.

Main duties of the job This is a 1 WTE Perm post You will be a core member ofBHFT supported discharge service You willas work as a key part of the MDT within both the acute and the community Providinga high level of clinical assessments and interventions as appropriate including indirect work as appropriate You willlaisse with other services within both primary and secondary care to ensure those individuals with more complex needs with in the acute have access to timely appropriate services to enable an integrated holistic approach.
