**What you’ll be doing**:
The primary responsibility of a Social Work Assistant is to work with our Slough residents in a strength-based and community-focused way. Through the process of assessment you will be asked to listen and understand what is important to the individual, explore their goals and, in the first instance, to assist them to identify what support they have already available to them in the community and through the use of Assistive Technology to achieve their desired outcomes.

You will be linking individuals or families with social and community services such as health, Income Boost and Community Connectors. You will also arrange meetings with relevant agencies and act as an advocate to help residents throughout their journey.

**You will need to have**:

- Ability to manage autonomously a caseload of service users with various disadvantages, assessing and treating them and maintaining their electronic records.
- To work collaboratively with health and integrated teams and acute colleagues, including Local Access Point and Discharge to assess teams.
- To work with other members of the MDT to ensure effective assessment, safety, treatment, management of care pathways and transfers of care / discharges.
- Excellent time management skills. Including the ability to work to deadlines
- Demonstrate positive attitudes and behaviours, taking a strength based approach to assessments
- Experience of working with people of diverse cultural and religious communities
