Community Mental Health Practitioner

Job summary We are looking for a Community Mental Health Practitioner with a social work or mental health, children's or LD Nursing background to join us in the CYP crisis and intensive support team supporting the Specialist Mental Health Practitioners and the wider MDT.This role will entail Delivering a high quality clinical service to children and their families.Care coordination of an identified caseload with access to regular and robust supervision.Delivering professions specific mental health interventions, in accordance with an agreed care plan, to an identified group of children and families experiencing mental health related difficulties.
Main duties of the job Take responsibility, with supervision from senior members of staff, for the management of a complex caseload, which includes, specialist assessment (within service specification), care planning in conjunction with child and family, delivering treatment interventions and evaluation of individual treatment plans (Care Programme Approach).Assess patient's individual care needs, develop, implement and evaluate programmes of care.Deliver a range of interventions in accordance with the agreed plan of care and service guidance including; pharmacological and psychologically informed interventions.Complete and contribute, where appropriate to robust risk assessments and risk management plans.

Person Specification Qualifications & Training Essential Registered Mental Health Practitioner/Nurse on NMC or Social Work England Register Conforms to NMC/Social work England requirements Desirable BSc degree in nursing or a mental health related subject or equivalent level Registered mentor/ supervisor Psychological intervention Qualification including EMDR, CBT, CBTp, Family Interventions, CAT, DBT Phlebotomy training NVR, Solution focus therapy, Positive Behaviour Support Planning, Experience Essential Experience of working with people experiencing mental health related difficulties.
Experience of working with people presenting with mental health needs in the community Experience of assessing, planning, co-ordinating and managing people with mental health problems in a community setting.
Ability to access any local resources and to use networking where possible.
Desirable Experience of working in community setting and making decisions autonomously Experience of working with children and families who have complex mental health and systemic needs.
Skills, KNowledge & Abilities Essential Skills of assessing, planning, treating and evaluating care, for people with mental health related difficulties.
Skilled in the assessment of clinical risk and developing and communicating appropriate risk management plans as part of the MDT.
Ability to demonstrate leadership skills - including delegation and supervision to junior staff.
Knowledge of Mental Health Act, the Children Act, Gillick Competency, Human Rights Act, Mental Capacity Act.
Knowledge of the physical health needs and monitoring requirements for Children and young people.
Skilled in providing lifestyle and health advice Knowledge of pharmacological interventions prescribed in mental health.
Knowledge of mental health evidenced based interventions for children and families.
Skilled in interventions in creative engagement.
Skilled in interventions to support concordance with treatment interventions.
Standard keyboard skills required for inputting onto RiO.
Demonstrate knowledge of evidence based practice and clinical governance.
Desirable Skilled in utilising specific assessments and outcome measures including; RCADS, ORS, Current View Knowledge of CYP-IAPT core principles.
Skills of assessing, planning, treating and evaluating care, for children and families with mental health related difficulties, utilising the principles of systemically driven care.
Skilled in undertaking and interpreting physical health investigations.
Personal Attributes Essential Able to fulfil the travel requirements of the post.
