The Role: As a Community Practitioner, you'll work closely with adults with care and support needs, their families, and carers.
Your aim is to make a positive difference by promoting independence, wellbeing, and informed choices.
Key Responsibilities: Deliver person-centred care and support tailored to individual goals and preferences.
Take a holistic approach, supporting families and carers to enhance the lives of those in their care.
Build strong community links and partnerships to provide effective support networks.
Conduct comprehensive assessments, create care plans, and review them regularly to meet personal outcomes.
Manage crisis situations and lead safeguarding enquiries.
Maintain high standards through personal reflection, supervision, and training.
Work flexibly across locations and teams, ensuring an effective, resilient service.
What Were Looking For: Qualifications: Degree or Diploma in a relevant field (e.g., Social Work, Nursing, or Occupational Therapy).
Up-to-date registration with your professional body.
Competence at the new practitioner level of the Social Care Capabilities Framework.
Experience: Post-qualification experience in social care, health, or related fields.
Expertise in risk management, safeguarding enquiries, and supporting individuals with medium to high-level complex needs.
Proven ability to work collaboratively in multi-agency environments.
Skills and Abilities: Excellent communication, mediation, and negotiation skills.
Strong IT and administrative capabilities.
The ability to manage workloads independently and as part of a team.
A commitment to promoting independence and innovative approaches to care.
Flexibility to travel and work varied hours to meet service needs.
Knowledge: Familiarity with social care legislation, safeguarding policies, and equality principles.
Understanding of available local resources and services.
Awareness of confidentiality, GDPR, and information governance requirements.
