Healthy Together offers a range of services to meet the varying needs of our service users.You must have a current NMC registration in any branch of nursing and a Specialist Community Public Health Nursing (SCPHN) Health Visitor registration.
As the ideal candidate you will demonstrate a warm, respectful, caring, and compassionate approach to delivering a flexible and responsive service for diverse and continually evolving health needs of our diverse population.
Main duties of the job To deliver Evidence Based healthcare, maintaining a high standard of professional practice.Lead and deliver the Healthy Child Programme, within a multi-agency team setting.Be responsible for the identification, assessment, and management of health care needs of individuals, families, groups, and communities, and for the development, implementation, and evaluation of programmes of care within a range of targeted groups.Provide delegated management responsibility to the Healthy Child Programme Practitioners and Healthy Child Programme Nurses.Mentor pre and post registration students, and new members of staff as required, and support induction programmes.You must have a full driving licence that allows you to drive in the UK and the use of a car for this role.
LPT operate 2 car schemes Car Leasing and Car Salary sacrifice subject to meeting the acceptance criteria.
