Job summary To provide clinical expertise, clinical leadership and line management to staff working in the Unscheduled Care Co-ordination (UCCH) Hub.To support staff to achieve and ensure competencies are maintained within the services.To contribute in the delivery of high standards of health care to patients within their own places of residence.To ensure that Trust annual priorities and quality goals are embedded within the workforce by ensuring that all annual appraisals are complete within the team.
Main duties of the job To provide professional and clinical leadership demonstrating a supportive and flexible leadership approach and a proactive responsible attitude so as to promote confidence in the team and Service.To work with and deputise for the Service Lead as required.To engage and lead on caseload management by overseeing, monitoring, and evaluating care plans for all patients to ensure quality care, treatment progression and timely discharge.To provide specialised overview of remote monitoring systems within the Virtual Ward & UCCH hub cohort across the sector, escalating clinical concerns promptly to the Place teams.To lead the triage of incoming referrals to UCCH providing a safe pathway into receiving care at home under UCCH.To work with all health care professionals, and statutory/non-statutory agencies to provide a seamless, integrated service to our service users.To support community clinicians and students through supervision, appraisal and mentorship in line with Trust policy.

Assess patients holistic needs, communicating highly complex and sensitive information to patients and careers as to their assessment, diagnosis, prognosis and treatment plan.
Become specialist in Assistive Technology and utilise it in patient care where appropriate.
Where there are barriers to understanding, such as hearing impairment, mental capacity impairment and other difficulties in comprehension, explanations require adjustments in order to gain understanding, consent and concordance.
Liaise with UCCH & Virtual Ward clinicians, GPs, Social Services, inpatient teams, other allied health care professionals and the voluntary sector to ensure identified needs are met and care coordinated appropriately.
To provide patient focused education, training and advice to patients, carers, other professionals, and voluntary organisations.
Provide clinical expertise and advice to others, including Social Services, GPs and other agencies.
Work with staff to ensure competencies are maintained.
To maintain clinical credibility by acting regularly as a clinical practitioner in own area of expertise.
Liaise with the Community Virtual Ward Clinical Lead Manager regarding patient care and referral activity into the hub.
Deputise in the absence of the Virtual Ward Clinical Lead manager.
Be required to undertake risk assessments and act upon them appropriately and beresponsible for ensuring workload for self and others is planned and prioritized according to service targets and needs.
Engage and lead on triage and referral to discharge standard processes.
Actively contribute to integrated team meetings and other meetings where appropriate and chair meetings as necessary.
Develop and maintain effective working relationships and communication with all members of the organisation, other professionals and agencies.
Instigate, and actively participate with the Clinical Operational Managers in the development of the operational strategy and project management.
Support all team members within the integrated team, respecting them, their roles and contributions.
Be aware of budget and ensure all effort is made to work within this.Participate in the recruitment process, using the opportunity to skill mix to meet the demands of the service.
Attend training as required to develop current job role.
Remain accountable for own professional actions as determined by the professional bodies.
Be accountable for work delegated to others within the team, ensuring staff are competent to undertake delegated working environment is conducive for learning and an appropriate learning experience for students.
To relay information from the Management team in respect of NCH&C and local issues to all members of the team on a regular basis in the form of business meetings.
To assist in the resolution of any conflicts, resolving informal complaints locally and assisting in the investigation of and response to formal complaints as required.
To participate in the staff appraisal scheme as both an appraisee and appraiser, maintaining a dynamic personal development plan.
To demonstrate a sound understanding of clinical governance and risk management, and apply to work situation, and ensure the team do likewise; identifying, assessing and documenting risk of implementing change.
To monitor and manage staff absence, mandatory training, annual leave and study leave in line with NCH&Cs policies, procedures and guidelines.
Inform the Locality Manager of any significant trends/issues.
To accurately complete personal human resources documentation in respect of team members.
Ensure that designated time frames are adhered to.
Complete all data returns as required by NCH&C.
Collate report, manage, and analyse information on activity levels and quality, in accordance with agreed performance standards.
To monitor the delegated team budget and assist the Locality Management Team to effectively manage resources.
Person Specification Qualifications Essential RGN with current NMC registration 1st level degree in Nursing or working towards degree Evidence of Continuing Professional Development and its application.
Mentorship preparation/ENB 998 Recognised Management Qualification or evidence of working at that level Desirable Masters degree Independent/ supplementary prescribing qualification Advanced Assessment qualification Leg Ulcer Management Qualification.
Experience Essential Experience of working at band 6 level or above within a community setting Evidence of clinical and professional leadership skills Desirable Experience of leading a team Experience of managing budgets Skills, Abilities and Knowledge Essential Knowledge of additional specialist areas through in-house training & short courses Excellent communicator and good interpersonal skills Ability to prioritise and manage stressful situations.
Highly motivated and a team player Able to provide high standards of care Tact & diplomacy and ability to talk to patients & careers and members of the health care team.
Empathy & sensitivity Ability to use own initiative Good observational & reporting skills Concentrate when undertaking patient care and inputting data/ patient records Will be able to make own travel arrangements to patients' homes, clinics, base and meetings etc., as required Work pro-actively to accommodate patient/service needs Be able to kneel, bend & stoop, and work in cramped environments Be able to manoeuvre limbs of around 5-6 kg Be able to manoeuvre patients using handling aids Have a good level of IT skills Desirable Be aware of Assistive Technology.
