Care Co-Ordinator

Job summary We have an exciting new opportunity for an enthusiastic team member to become a Care Co-ordinator based at The Colte Partnership head office in Feering.
The role is part time - 20hrs per week, and will initially be for a 3 month contract.
The role will be to work on a special project, working with patients to carry out holistic assessments.
The Care Coordinators role will support the primary care professionals in coordinating all key activity including access to services, advice, and information, and ensuring health and care planning is timely, efficient, and patient-centred.
The role will include supporting digital initiatives and includes responsibilities for the co-ordination of the patients journey through primary care.
Interviews will be carried out for successful applicants on receipt of CV and covering letter, and we reserve the right to retract the advert if a suitable person is found before the end date.
Main duties of the job As a care coordinator, you will be involved in proactive, person-centred care planning with people, overseeing and coordinating their care when needed, by bringing together the different specialists whose help the individual may need.The Care Co-ordinators responsibilities include but are not limited to the following: Organising care for patients as requested.
To help people to manage their needs through answering queries, making and managing appointments.
Use healthcare technologies to optimise service delivery, patient access and continuity of care.
