The role includes working with individuals to understandtheir health needs and to plan appropriately for these.
The role will coverholistic care which includes both physical and mental health aspects.
Casemanagement and coordination of support, assessing and reviewing the needs in aperson centred approach.
Liaising with other agencies and providers to ensurereasonable adjustments are in place.This role may not be eligible for sponsorship under the Skilled Worker route, please refer to the Direct Gov website for more information with regards to eligibility.
Main duties of the job As part of the multidisciplinary team you will be responsible for assessing, planning and managing risk of individual's care needs including physical, mental health and behaviour that challenges services.Case management and care coordination for those that are fully health funded.
Liaising with commissioners in conference regarding the identified health needs and how they are being addressed.Speaking up for individuals with a learning disability and ensuring reasonable adjustments are made by generic services to allow the service user to have equitable health care by the most appropriate service.Liaising with other agencies, providers and families in partnership with the individual and to keep in mind the safeguarding process.Ensuring consideration of capacity, restrictive practice in line with best interest and court of protection processes.To be an autonomous practitioner and be able to prioritise workloads to meet the demands of the service and the wider Livewell SW objectives.Please note that Livewell may close the job advertisement earlier than the specified deadline if a high number of applications are submitted.

Communications and relationshipsThepost holder will be required to work within the MDT function of the CLDT andmay need to be part of wider MDT groups relating to client need.Thepost holder will liaise between other health, social and educational servicesin assessing, planning and reviewing client care needs/service provision.Thepost holder will also work with other voluntary and secondary care providers.
Person Specification Qualifications Essential DipHE Nursing and or Degree Professional Registration.
3 years Post registration experience.
Evidence of continuing professional development.
Evidence of managing a caseload of complex individuals.
Desirable Mentorship Certificate.
Leadership qualification/knowledge Knowledge Essential Knowledge of Continuing Health Care (CHC) processes Knowledge of the Mental Health Act.
Able to demonstrate understanding and competency in assessment, care planning and review.
Good knowledge and understanding of the Mental Capacity Act, Deprivation of Liberty and reasonable adjustments.
A good understanding of the safeguarding adult process and relevant issues.
Able to problem solve and be creative in care planning Experience Essential Working in multidisciplinary teams and across health and social care.
Managing a complex caseload of individuals who pose a risk to themselves and society.
Experience of working across organisational and inter-agency boundaries.
Experience of working with people with learning disabilities / mental health issues whose behaviour poses a risk.
Desirable Experience in mentoring students.
Working in close contact with the ICB and local commissioners Additional Requirements Essential Full driving license is required as the post holder will be travelling out of area.
The ability to be away from home overnight is an integral part of the role.
