Job summary We are lookingfor skilled Registered Nurses/Clinicians to be part of the Rapid Access to CareTeam.
This will include the holistic assessment of patients to support them tomake decisions about the place of care they would prefer at the end of theirlives, the completion of relevant documentation, and the smooth transfer fromone care environment to another.

Job description Job responsibilities As a RADS Clinician you will work within a multi-disciplinary team providing clinical care,navigation,coordination and support to hospice patients and their families for patients who are thought to be in the last three months of life Through holistic assessment provide information, advice, psycho social and spiritual care to patients with palliative and end of life care needs, supporting their families and carers, liaising and referring on as necessary to appropriate health and social care professionals to enable them to receive care in the location preferred at the end of their lives.
To work in partnership with those receiving care, helping them to access relevant health and social care, information and support when they need it.Support people to access, identify, appraise and interpret information, to enable them to determine their own future.
To maintain effective communication skills to deliver highly emotive information clearly, sensitively and unambiguously with staff, patients, families and professionals by face-to-face, telephone and/or electronic means.
To work collaboratively as part of the multi-disciplinary team (MDT)sharing the clinical caseload of patients/carers within the locality/in-patient unit for patients who have palliative care needs to ensure efficient and timely care is coordinated and delivered in order to make sound decisions around palliative and end of life care.
To work collaboratively with the locality MDTs to ensure seamless care for the patients who require admission, discharge or transfer between settings in a timely way .
To work in collaboration with the patient, their family or carer and other agencies to ensure placement where possible can meet the patient's preferred place of care/death.
To coordinate and negotiate placements with domiciliary care agencies and Nursing homes, and oversee ongoing monitoring and review of such placements in accordance with FH service specification and in liaison with FH Finance team.
To support management in training and mentoring a multidisciplinary workforce from novice to expert to work as one team.
To support any streamlined locality/IPU processes and services by embedding quality improvement methodology including clear outcome measures to produce efficient and highly performing community services.
To support the Hospice in building strong relationships with external partners and stakeholders and promote the good reputation of the Hospice trusts, CCG,health and social care partners, businesses and voluntary organisations.
To support and guide Health Care Assistants,Healthcare Support Workers and volunteers in their daily role model the values of Farleigh Hospice and support others within the organisation to do so by recognising positive behaviours, challenging poor behaviour, encouraging and supporting change.To be visible and credible with the ability to listen and respond to staff and to patients and family issues and insights.
To communicate effectively with all staff across the organisation to promote one team, one vision and collaborate with colleagues as appropriate to deliver this.
To work as part of a team and provide clinical cover during the absence of other team members.
To work as part of a team to ensure,the priorities of the service are met especially in times of high demand.
To adapt to the pressures of changing demands day to day.
To participate in the development of a Hospice response to any identified changes in the supportive and palliative care needs of our population and engage with patients, carers and external professionals to positively share any agreed change in service offer To recognise the needs of individuals within the team and be able to provide support and guidance to them To demonstrate skill in conflict resolution and use competent negotiation skills when dealing with difficult or challenging situations such as managing patient/carers unmet expectations.
