Our list size is approximately 18,500patients.
Main duties of the job You will be part of a team responsible for ensuring the delivery of safe and effective nursing care to the whole practice population.
You will be required to work across our two sites to ensure service delivery is available to all patients.The post holder will ideally have qualifications and experience in the diagnosis, management and treatment of diabetes, or an interest and willingness to specialise in this area.
The successful candidate should ideally be able to undertake cervical screening and baby immunisations alongside general nursing procedures.
We are looking for a non-medical prescriber or someone who is willing to work towards this qualification.
We can be flexible with hours and offer either full time or part time for the right candidate.
About us We are a large, forward-thinking GMS training practice operatingover two sites, involved in the training of GPs, medical students, nurses,paramedics and overseas doctors.

Job description Job responsibilities Clinically, the focus of the role is the delivery ofevidence-based practice for patients with acute and chronic conditions, and providing management and preventative nursing interventions to all patients.
You willwork collaboratively with the whole general practice team to meet the needs ofpatients, supporting the delivery and procedures under the direction of theLead Nurse.You will provide high quality, holistic care to patients in a safe, multi-disciplinaryenvironment through the provision of specialist clinics, chronic diseasemanagement, cervical smears, child/adult immunisations and health education.NMC requirements must be met and the individual will work within the NMC Codeof Conduct to agreed practice standards and protocols.

Experience of working in General Practice Holding or working towards qualifications in diabetes Desirable Minimum 2 years post registration.
Holding or working towards qualifications in chronic disease management.
Computer skills (EMIS preferred).
Post-registration study in topics allied to general practice.
Experience Essential Experience & interest in education within a primary care setting.
Desirable Good interpersonal skills, both verbal and written.
Evidence of problem solving and decision-making skills.
Good team player.
Ability to be reflective, accept criticism and act constructively.
Willingness to accept additional responsibilities.
Gets on well with people at all levels.
