About the Role

We are seeking an experienced and skilled pharmacist to lead our Home Oxygen Service in South East London. As a key member of our team, you will be responsible for delivering high-quality patient care and ensuring the safe and effective management of oxygen therapy.

Key Responsibilities
  • Lead the development and implementation of medicines optimisation strategies to improve patient outcomes and reduce healthcare costs.
  • Collaborate with healthcare professionals to ensure seamless care transitions and effective communication.
  • Provide clinical leadership and guidance to pharmacy teams to ensure the delivery of high-quality patient care.
  • Develop and maintain relationships with stakeholders to ensure the effective delivery of oxygen therapy services.
  • Contribute to the development and implementation of policies and procedures to ensure compliance with regulatory requirements.
Requirements
  • Master's degree in pharmacy or a related field.
  • Current registration with the General Pharmaceutical Council.
  • Postgraduate qualification in clinical or community pharmacy or equivalent experience.
  • Mandatory Continuing Professional Development to maintain fitness to practice and revalidation.
  • Experience of developing and implementing medicines optimisation services in a challenging environment.
  • Knowledge of developing long-term conditions prevention or management services and an understanding of personalised care.
  • Extensive experience of negotiating and implementing change in practice.
  • Experience of managing the delivery and improvement of services within a complex and diverse organisation.
