About the Role

We are seeking a highly skilled and compassionate Social Worker to join our Hospital Pathways Team in Adult Social Care. As a key member of our team, you will play a vital role in empowering adults to live independently and make informed decisions about their care.

Our team is dedicated to providing high-quality care and support to individuals with diverse needs, and we are looking for someone who shares our passion for delivering person-centred care. You will work closely with our NHS colleagues to facilitate safe and timely discharge, enabling individuals to return to their own homes and regain independence.

About You

We are looking for a qualified Social Worker with current registration with Social Work England and eligibility to work in the UK. You will have a minimum of 2 years post-qualifying experience working in adult services, assessing and supporting individuals with diverse needs, and successful completion of your ASYE.

You will possess sound knowledge of relevant legislation, including the Care Act and Mental Capacity Act, as well as social work theories. Strong assessment, outcome-focused care planning, and risk management skills are essential for this role.
