Job Summary:

Cera Care Ltd is seeking a skilled Care Coordinator to manage the delivery of high-quality care services. The successful candidate will effectively match Care Workers with Clients based on their care needs, ensuring efficient and effective care delivery.

Key Responsibilities:

  • Coordinate the delivery of care services to meet Client needs
  • Match Care Workers with Clients based on care requirements
  • Monitor and evaluate care service delivery
  • Develop and implement care service improvement plans
  • Collaborate with Care Workers and other stakeholders to ensure effective care delivery

Requirements:

  • NVQ/SVQ 2 or 3 qualification (desirable)
  • Registration with the SSSC (essential)
  • 2 years experience working as a care coordinator for a Domiciliary Care Agency (desirable)
  • Knowledge of CQC/CI/CIW Compliance (essential)
  • Excellent communication and organizational skills
  • Ability to work flexibly and as part of a team
