About the Role

We are seeking a highly skilled and compassionate Care Coordinator to join our Virtual Ward team in East Sussex. As a key member of our dynamic team, you will be responsible for coordinating care for patients across our virtual ward pathways.

The ideal candidate will have experience in remote patient monitoring and be passionate about delivering high-quality, patient-centered care. You will work closely with our multidisciplinary team to provide leadership and support to ensure seamless care delivery.

Main Responsibilities
  • Lead the onboarding and clinical management for patients on remote monitoring
  • Develop your clinical skills in conducting remote clinical assessments and treatment for patients across the Virtual Ward pathways
  • Support the clinical coordination of the team on shift
  • Provide leadership to the team on shift and support the development of the service through leading on projects and audits

We offer a competitive salary range of £37,338 to £44,962 per annum, pro rata, depending on experience.
