Care Coordinator for Adults

Nurseplus is a leading provider of mental health services, seeking an experienced Care Coordinator for Adults to join our team. As a Registered Mental Health Nurse, you will be responsible for coordinating care for adults with complex mental health needs.
- Care Coordination: Develop and implement individualized care plans to address patients' specific needs.
- Therapeutic Support: Provide one-to-one counselling, group therapy, or cognitive behavioural therapy (CBT) to promote emotional well-being.
- Team Collaboration: Work with multidisciplinary teams to deliver comprehensive and compassionate care.
- Patient Safety: Assess and monitor at-risk patients for signs of suicidal ideation or self-harm, intervening swiftly to ensure safety.
