Job Title: Specialist Multiple Disadvantage Practitioner

Job Summary:

Central Recruitment Services Ltd is currently seeking a skilled Specialist Multiple Disadvantage Practitioner to join our team in the Westminster area. The successful candidate will be required to work 37.5 hours per week, Monday – Friday, 9AM – 5PM.

Key Responsibilities:

* Hold a caseload of complex needs clients and conduct regular key working sessions resulting in recovery plans that define the basis for further interventions
* Manage referrals within agreed timescales, both in-house referrals of existing service users and by supporting new scheme referrals via specialist input when deemed appropriate
* Conduct skilled and sensitive assessments with clients who may be ambivalent about taking-up support, and who may have had difficult prior experiences with specialist services
* Participate in the delivery, evaluation and development of the in-house recovery programme
* Build and maintain effective relationships with all stakeholders, as well as the client
* Convene and/or attend regular multiagency treatment reviews to support the progression of multi-modality care plans
* Provide specialist support and advice to practitioners at the service to enable effective progression of treatment plans

Requirements:

Working Hours:

37.5 hours per week, Monday – Friday, 9AM – 5PM

Pay Rate:

£15.56 per hour, with £1.88 holiday pay, totalling £17.44 per hour

Duration:

Temporary cover required for approximately 2-3 months
