Job Summary

We are seeking a skilled Community Nurse to join our team in Aberystwyth, Ceredigion. As a Community Nurse, you will play a vital role in providing high-quality care to patients in their own homes, working closely with the District Nurse Team Leader and other healthcare professionals.

Main Responsibilities
  • Manage a delegated caseload, providing complex care management to patients in their own homes.
  • Ensure high standards of care are delivered to patients and their families/carers, liaising with staff, patient/carers, and other agencies/organisations as required.
  • Travel between sites in a timely manner, utilizing your ability to communicate effectively with patients and colleagues.
  • Participate in the development of community practice, aligning with current health policy and promoting professional growth.

Requirements
  • Level 1 Registered Nurse (Adult)
  • Current NMC Registration
  • Evidence of personal continuing development, statutory, and mandatory training
  • Awareness of Health Board policies, procedures, and guidelines
  • Ability to relate theory to practice
  • Understanding of primary health care working
Desirable Qualifications and Experience
  • Postregistration experience in acute services
  • Teaching and assessing
  • Understanding of clinical governance, health & safety, risk management awareness
  • Community experience
  • Willingness to undertake further training as necessary
  • Evidence of post-registration education - Diploma/ Degree Level
Language Skills

The ability to communicate in Welsh is desirable for this post. If you do not meet the Welsh Language requirements specified, the Health Board offers a variety of learning options and staff support to help you meet these minimal desirable requirements during the course of your employment with us.
