Job Summary
We are seeking a skilled and dedicated Community Nurse to join our team at Hywel Dda University Health Board.
As a Community Nurse, you will be responsible for providing high-quality care to patients in their own homes, working closely with the Community Nursing Team to deliver effective care management and support.
Main Responsibilities
Key duties of the role include:
- Providing comprehensive nursing care to patients in their own homes
- Working collaboratively with the Community Nursing Team to develop and implement care plans
- Liaising with other healthcare professionals and agencies to ensure seamless care delivery
- Conducting assessments and evaluations to determine patient needs
- Developing and implementing individualized care plans to meet patient needs

Requirements
To be considered for this role, you will need to have:
- Level 1 Registration as a Registered Nurse (Adult)
- Current NMC Registration
- Evidence of personal continuing development and statutory training
- Ability to relate theory to practice and work effectively in a community setting
Desirable Skills
We welcome applications from Welsh speakers, although the ability to communicate in Welsh is not essential for this role.
